of application does NOT ensure exceptance into the association, only after presentation of skills to the board of CFMA is
the applicant advised on acceptance or needed improvements prior to acceptance.
Welcome to the Shaolin Chuan Fa Martial Arts Association
By filling out the following application you are on your way to opening the door to growth, knowledge, and friendship.
This applicaiton is the first step to acceptance into the CFMA (Shaolin Chuan Fa Martial Arts Association). To ensure
continued high training standards of the CFMA each applicant is required to submit verification of program and lineage. Such
submitted information simply verifies formal training and DOES NOT prevent the "break away" program from joining
the association. The following requested information must be verifiable, and further requested material may be required
before final evaluation may be made by the board of CFMA. Final evaluation of an applicant program is outlined in the
Chuan Fa Association & Guidelines Booklet but is summarized here as: Application
process and verification of applicant. Upon completion of application process, applicant submits
abilitiy of skills via video, DVD, or in person for board evaluation of technique and training methods. CFMA votes on applicants acceptance into association Upon majority acceptance
by the CFMA board, applicant is notified of acceptance; requiring finalization of application costs, and commitment

be paid by check, or money order, to Dr. Dennis Fitch, unless a specific processor is listed for region
